When the form has downloaded, please print and complete the information sheet and bring it to the hospital at the time of your appointment to recieve a discount off of your first visit with us.

Please note: for the safety of our staff and other pets, your dog must be up-to-date on Rabies, Distemper Combo and Bordatella vaccines, and your cat must be up to date on Rabies and Feline Distemper Combo vaccines before being groomed at North Paws Veterinary Clinic. Pets cannot be groomed within 24 hours after receiving vaccinations due to possible vaccine reactions within that period of time. For more information, please contact North Paws Veterinary Clinic.
